Submit your advisor email address, Advisor CRD #, and Firm CRD #. The request is checked against the public SEC AdviserInfo/IAPD JSON service before the access link is emailed to the advisor.

Use this form to receive the ETF possibilities link by email. The advisor CRD and firm CRD must both verify, and the advisor record must show the submitted firm CRD. 5thN Financial is BCC’d on verified access messages for request tracking.
By submitting, you are requesting access to ETF-indexing information and authorizing 5thN Financial to receive a copy of the access request details, including your email address, advisor CRD number, firm CRD number, and the date/time of the request. Verification depends on the availability of the public AdviserInfo/IAPD service. Detailed IAPD verification data is retained only in the internal 5thN copy and is not sent to the requester.
